To Become a New Patient…

• Fill out the Child Evaluation Form  This form will be emailed to our office when you click the “Submit” button.
  
  
• Our intake nurse will call you within approximately five business days.
  
  
• After speaking with the intake nurse, please call our office to give us your name, the patient's name, and the phone number where we can reach you to schedule an appointment.
  
  
• We typically make calls to schedule first appointments on Friday afternoons.
  
  
• The Child Evaluation Form must be completed and submitted to our staff before scheduling your first appointment.  If you have any questions, please email jessica@icdrc.org or call 321.259.7111.
  
  
• We have prepared the following information to answer many of your questions and to better prepare you for your appointment with us.  Please take a few minutes to review the entire packet and save it for future reference.  The legal forms must be completed and returned to our office with your medical records prior to your appointment.
